General requirements for most Surrogates:
(clinics can make independent decisions based on last birth, current health, # of C-sections,)
Be between the ages of 20-42
Have given birth to at least one child of her own
Have had uncomplicated pregnancies and deliveries
Be height-weight proportionate (each clinic has their own BMI limits, some require under 35.0, some are happy with under 34, 32, and 30. Typically TS's are under 36.0

Be in a stable living situation
Have the support of her spouse or partner (when applicable)
Be a citizen or permanent resident of the United States
Be willing to take medications via injection
Not smoke or have used illegal drugs, or be exposed to second hand smoke
Be willing to refrain from alcohol throughout the pregnancy
No criminal history of child abuse, animal abuse, DUI/DWI, or drug charges
Not have any psychiatric illness and able to pass a psychological evaluation
Have good communication skills
Be reliable, responsive, and mature
Have reliable transportation to travel to doctor’s appointments and screening
Be able to provide contact information for past OBs or family doctors
Enjoy pregnancy and be motivated by the wish to help others create or add to their family
Acceptable psychological evaluation. We will help you obtain an evaluation. If you've previously failed a psychological evaluation you will not be able to be approved.
At least one prior, uncomplicated term delivery. No more than 3 previous cesarean sections.
Recommend no more than 6 total previous deliveries-provider discretion. Non-smoker.
Immune to varicella/rubella. We will help you obtain this testing if you are unsure of immunization status.
No medications Category C or greater.
No previous obstetrical history of the following:
Severe proteinuria
Preterm delivery
Placental abruption
Gestational hypertension
Superimposed gestational hypertension
Preeclampsia or Eclampsia Severe thrombocytopeni a H.E.L.P.disorder
Vasa previa
Postpartum hemorrhage
Disseminated lntravascular Coagulation
Preterm labor requiring medication to stop contractions
Deep vein thrombosis
Preterm premature rupture of membranes Intrauterine fetal demise
Intrauterine growth restriction Hyperemesis
Placenta previa Uterine rupture Oligohydramnios Polyhydramnios
Gestational Diabetes Shoulder dystocia lntrahepatic cholestasis Recurrent pregnancy loss
Structural abnormality of cervix or uterus
Pulmonary embolus
Seizure or Stroke in antepartum, intrapartum, postpartum period Severe postpartum depression or postpartum psychosis
Previous C section with T incision
No medical history of the following:
Hypertension
Unmanaged/untreated cervical dysplasia
Infectious disease (particularly CMV in a previous pregnancy,TB, HIV, (HPV, HSV ok)
Greater than or equal to three cone biopsies on the cervix or two LEEP procedures
